Peace of Mind Program

For long distance caregivers with an aging loved one in the Fredericton area

 

Do you live away from Fredericton now, but have an aging loved one here that you constantly worry about?

 

Do your career and family responsibilities keep you from returning home to check on your aging parent in Fredericton as often as you would like to?

If you answered yes to any of these questions, then we can help. Our Peace of Mind Program has been specifically designed for long distance caregivers.

 

We begin with our RoadMap to Success Program. Initial phone consultation, in home assessment, and a written care plan customized to the needs of your loved one, including recommendations for local resources and supports.

 

But we don’t stop there.

 

Our Peace of Mind program offers a local, on the ground professional presence for YOU and your family. A Care Manager provides ongoing, supervision of your loved one, including scheduled home visits and telephone calls to check on safety and wellness, to provide socialization and meaningful conversation, and to ensure that any services in place are working well. A detailed outline of the recommended Peace of Mind services will be included in our written care plan.

 

As we move forward your Care Manager will communicate with you regularly via phone, email or video call. It is designed to be a cooperative and collaborative effort to support YOU and your loved one; to encourage them to be Healthy@Home, for as long as possible and to give YOU the Peace of Mind you need and deserve. You can rest a bit easier knowing your loved one is in good hands here in Fredericton!
